“We linked the app to clinical results,” Bosch says. “Patients have improved glycemic control and do not go to emergency rooms frequently.” According to Rivera, patients with the most complex cases of chronic diseases enrolled in the Cynara Civic Population Health Management Program had a 23% reduction in emergency room visits, 18% decrease in readmissions within 30 days, and 18% decrease in readmissions within 30 days. Patients were involved in technology-enhanced clinical programs for at least 12 months. Ribera compared the level of patient use of services before and after the program.

More broadly, Cynara citizens, developed by Rivera with Microsoft Technology, reduce the gaps in care, help patients follow their health plans, and are less likely to have to go to the emergency room. For example, by looking in real time whether diabetics have lower glucose levels, Rivera's health professionals can call patients to respond. Because patients know who is in their care team and can send messages in addition to making appointments, Rivera believes patients feel they have access to care and good access, Bosch says. The app runs on Microsoft Azure, making it easy and secure to access it from almost anywhere.
